How to Make a Referral

There are two ways to refer for Counselling with Wellelder.

1. If You're a Health Professional or Support Worker

Refer an older adult to WellElder by completing the form below:


2. If You're Referring Yourself

If you're an older adult seeking support for yourself, complete the form here:


What Happens After You Submit a Referral

  • We’ll review the referral and get in touch to arrange next steps.
  • Our office is open Tuesday to Thursday, and we aim to respond as soon as possible.

Keeping Referrers Informed

If you are referring someone else (and they give consent), we’ll keep you updated by letting you know:

  • When their first appointment is booked
  • When counselling has been completed
  • If they decide not to continue with counselling
  • Any updates they consent to for their ongoing care
  • Any follow-up you requested

If any concerns about safety or wellbeing arise during counselling, we will always contact you directly.
